LSA) Lichen Sclerosus Atrophicus) היא מחלה נדירה המערבת בדרך כלל את אזור איברי המין. בספרות הרפואית מדווח על פרשות חולים ספורות שלקו ב-LSA מפושט. מעורבות כפות הידיים נדירה אף יותר.

למרות שההסתמנות הקלינית ברוב החולים המתוארים היא טבה, תיתכן הופעה מפושטת של הנגעים, כוויצות (contractures) מיפרקים מגבילות ושאתות בעור. מובאת במאמר הנוכחי פרשת חולה עם LSA מפושט שהופיע בעשור השישי לחייו. ההסתמנות הקלינית הפכה לשאתות מסוג שאת תאי קשקש (SCC) מפושטות בגפיים העליונים ובאזורי גוף אחרים. בנוסף, הופיעו כוויצות מגבילות וקשות באצבעות כפות הידיים שמנעו מהחולה לבצע פעילויות יומיומיות.

אנו סבורים, כי בהסתמנות קלינית של LSA מפושט המערב את כפות הידיים נדרש טיפול תוקפני (אגרסיבי), על-מנת למנוע סיבוכים בעתיד.

Videothorascopic Sympathectomy for Palmar Hyperhidriosis

Palmar hyperhidriosis is not a life-threatening disease but leads to loss in the quality of life. Conservative treatment is ineffective and major surgery involves perioperative complications and esthetic impairment.

From 1992 to 1998 we treated 156 patients with palmar hyperhidriosis using a single port, drainless videothoracoscopic procedure with almost no complications.

Osteoid Osteoma of the Hand: a Rare Location

Osteoid osteoma is a benign bone tumor. It is rare in the hand where it may cause local swelling and pain. Marginal resection is almost always curative, without residual functional disability. Because it is rare in this location, osteoid osteoma is not usually considered in the differential diagnosis of painful lesions of the hand, which may delay treatment. Osteoid osteoma in the hand has characteristic clinical, roentgenologic and scintigraphic features. Early diagnosis of this lesion may be improved by recognition of these features. An algorithm for decision-making that may help is proposed. We describe our experience in 3 cases involving, respectively, the capitate bone, a proximal, and a distal phalanx, in which cases marginal resection was curative.
