        Video-Assisted Thoracic Surgery: Experience with 586 Patients

        Recent advances in optics, video systems and endoscopic operating instruments have led to increasing application of thoracoscopic surgery, as it has become easier to perform and more accurate.

        We performed 586 video-assisted thoracic surgical procedures for diagnosis and treatment (May 1992-Dec. 1998) 127 were for diagnostic thoracoscopy and 79 for pleurodesis. 380 cases of operative thoracoscopy included pulmonary wedge resection (for interstitial lung disease, benign and malignant pulmonary tumors and pulmonary metastases) bullectomy, management of empyema, pleural tumor biopsy, thoracic sympathectomy, pericardial window formation, thoracic spinal procedures and resection of posterior mediastinal cysts. Recently we have had good experience in evacuating blood and blood clots from the thorax which accumulated after cardiac and thoracic surgery.

        Patients were placed in the lateral thoracotomy position and were ventilated with a double-lumen endotracheal tube, enabling collapse of the operated lung. The operating approach was through 1-3 thoracic ports. Mean operation time was 55 minutes, chest-tubes remained for 2.2 days (mean) and mean hospitalization was 3.3 days. There were no wound infections or significant postoperative complications. 5 patients had air leaks longer than 7 days; none required further surgical intervention.

        There was intercostal neuralgia and Horner's syndrome after thoracic sympathectomy (1 each) In cases in which localizing the parenchymal lesion was difficult, the lung was palpated directly by inserting a finger through a small incision or a mini-thoracotomy. Conversion to thoracotomy was performed when primary malignancy of lung was diagnosed by frozen section. Only 2 patients had thoracotomy for uncontrolled bleeding. Thoracoscopy is a minimally invasive surgical technique with very low morbidity and high diagnostic accuracy. Postoperative recovery is brief and uneventful.

        Alibert היה הראשון שתאר בשנת 1806 התהוות ריקמת צלקת משגשגת כשאת ממאירה, המונח קלואיד (keloid) נגזר מהמלה היוונית chele שמשמעותה צבת הסרטן המדמה את הצלקת הקלואידית כנוגסת בתוך העור הבריא וסביבו.

        Mancicni וחב' הגדירו בשנת 1962 ו- Peacock בשנת 1970 את הצלקת המשגשגת כצלקת היפרטרופית – צלקת מורמת מהעור המתוחמת לגבולות הצלקת. קלואיד היא צלקת מורמת המתפשטת מעבר לגבולות הצלקת. בנוספת צלקת היפרטרופית עוברת תסוגה עצמית עם הזמן ואין הישנות שלה לאחר כריתה ניתוחית; אין הדבר כך לגבי צלקת קלואידית.

        האירגון האמריקאי לסוכרת (The American Diabetes Association - ADA) – ממליץ, שאם לא הושג איזון של הסוכרת בחולים NIDDM תוך שלושה חודשים על ידי תזונה דלת-סוכר והתעמלות, יש להתחיל בטיפול תרופתי. בדרך כלל מומלץ להתחיל בתרופה פומית אחת המורידה את רמת הסוכר, כמו, לדוגמה, (sSLP) sulfonylureas, (MTF) metformin, (ACS) acarbose או אינסולין.

        בבחירת התרופה, יש לדעת את הפתוגנזה העיקרית שהביאה להיפרגליקצמיה, מנגנון הפעולה של התרופה, מחירה, השפעות-הלוואי, אופן מתן התרופה ומידת הדחיפות להורדת רמת הסוכר, דהיינו, ההסתמנות הקלינית של החולה.

        התרופות מקבוצת ה- sulfonylureas הן השימושיות ביותר היום כטיפול התחלתי. Metformin מקבוצת ה- biguanides הוא תרופה מהקו הראשון לטיפול בחולי NIDDM הלוקים בהשמנת-יתר ועמידות לאינסולין. התרופה יעילה במיוחד במישלב עם תרופה מקבוצת ה- sulfonylureas, במיוחד במצבים בהם ה- sSLP בלבד לא הביאו לאיזון החולה.

        התרופה החדשה יחסית acarbose מקבוצת האלפא glucosidase מפחיתה בצורה ברירנית את עליית הגלוקוזה לאחר הארוחה, ואין לה כמעט השפעה על רמת הסוכר בצום. שתי הקבוצות שהוכנסו לשימוש רק לאחרונה הן:

        קבוצת ה- thiazolidinedione (troglitazon, rosigltazon) מגבירה את רגישות הרקמות ההיקפיות לאינסולין, כלומר, "מעוררת" תגובה חזקה יותר לאינסולין בשריר, בריקמת השומן ובכבד.

        קבוצת ה- meglitinides (repagilinide) מגבירה את הפרשת האינסולין מהלבלב בדומה לקב' ה- sSLP, אלא שהשפעתה מהירה יותר וקצרה יותר, ומתאימה להפחתת רמות הגלוקוזה בנסיוב לאחר ארוחה.

        Prevention and Treatment of Procedural Pain in Pediatric Wards

        Present knowledge and available pharmacological agents allow for adequate prevention and treatment of pain in children. We present guidelines we prepared for the prevention and treatment of procedural pain in children in our general pediatric ward. This followed extensive review of the literature, participation in scientific meetings, discussions with experts and consultation with interested clinicians. Successful implementation of the guidelines requires increased appreciation of the importance of pain prevention, participation of the nursing, as well as medical staff, and ability to evaluate pain in children of various ages.

        Surgical Repair of Myxomatous Mitral Valve

        Myxomatous mitral valve disease is now the most common cause of mitral regurgitation in the western world. Repair of the leaking valve has become standard surgical procedure during the past 2 decades. Between 1993-1999 we performed 113 repairs of the mitral valve. In 25 patients the etiology was myxomatous degeneration (no mortality). Long-term clinical results depend on patients' functional class at surgery. Based on this fact, and the good surgical results, it is recommended to refer such patients even with severe mitral incompetence for surgery at an early stage, even if symptoms are minimal.

        Diagnosis and Therapy in Acute Stroke: A Rehabilitation Center Viewpoint

        The extent of the diagnostic work-up of patients with acute stroke was evaluated in 101 patients admitted for rehabilitation during a 4-month period in 1997. This included specific blood tests and neuro- and cardiac imaging, and compared the extent of work-up in a community hospital versus a rehabilitation center. Comparisons were also made with similar investigations 10 and 20 years earlier.

        Results demonstrated that the trend to admit younger stroke patients (<50 years) to neurological (as opposed to medical) departments observed between 1977-1987 persisted in 1997.

        The use of CT scan increased dramatically from 1977 to 1987 (19% vs 78%), and in 1997 was actually 100% The use of carotid duplex and echocardiography increased steadily during the 3 decades reaching 26% and 28% respectively. Tests for thrombophilia were seldom done. However, in neurological departments it was done in about 50% of the younger stroke patients. In neurology departments carotid duplex was done 2 to 3 times more often than in medical departments.

        During rehabilitation imaging tests were done once or more in almost half the patients. The results and those of additional blood tests, have led to modification of antithrombotic treatment in 14% of the younger group and 4% of the older group.

        We have clearly shown that while stroke work-up has become more comprehensive in recent years, there is still much to do in this field. Stroke units or teams in our general hospitals will increase stroke awareness, improve work-up and hasten definitive treatment.

        Surgery for Blepharoptosis in Muscular Dystrophy

        In some muscular dystrophies there is ocular involvement characterized by blepharoptosis and ophthalmoplegia. These conditions occur in chronic progressive external ophthalmoplegia, oculopharyngeal muscular dystrophy, mitochondrial myopathy, myotonic dystrophy, and ocular myasthenia, among others. Although they differ in their systemic clinical manifestations and in genetic inheritance, ocular involvement is common to all of them. Manifestations include bilateral progressive blepharoptosis with or without extraocular muscle malfunction.

        During surgical repair of the ptotic eyelid, consideration must be given to eyeball movements, in addition to maximal eyelid elevation, and to avoiding overcorrection and consequent corneal overexposure, leading to dryness and visual impairment. With these muscular dystrophic disorders, resection of the levator muscle or blepharoplasty alone does not suffice. Follow-up shows that most patients need a secondary repair after a short while. Operative correction uses a frontalis sling for eyelid elevation and support.

        A series of 8 patients with these diseases, operated on by various surgical techniques during the past 7 years, is presented.

        A Molecular Method of Diagnosis of Congenital Adrenal Hyperplasia

        Congenital adrenal hyperplasia (CAH) is caused mainly by deficiency of the 21-hydroxylase enzyme. The disease may appear in the classical salt-losing, simple virilizing forms or as a mild, nonclassical form. 21-hydroxylase is encoded by the CYP21B gene on the short arm of chromosome 6, in the midst of the human leukocyte antigen (HLA) complex, between HLA Class I and Class II regions.

        We describe a method for identifying mutations in the CYP21B gene. It is based on amplification of the gene using the polymerase chain reaction and identification of mutations with sequence-specific oligo-probes. The mutations identified were: V281 and P30L responsible for nonclassical CAH, and I2 splice, Q318X, I172N, cluster E6, and a deletion including 8bP in the third exon (8bP del) responsible for the classical form of CAH.

        We also analyzed 2 families affected with the classical form of CAH which demonstrate possible complications in genotyping. Typing for HLA haplotypes can be helpful in certain cases, as demonstrated in 1 of the families presented. In this case it was necessary to distinguish between 2 possible genotypes: 1 with the mutations in tandem on 1 chromosome and the other with the mutated genes on both chromosomes. HLA haplotyping enabled the assignment of the mutations to the relevant chromosomes and thus allowed correct genetic counseling.

        The other family demonstrated the importance of CYP21B genotyping in individuals with the nonclassical form of CAH. This form may consist of 1 mild and 1 severe mutation, representing a serious potential for transmitting the classical form of CAH.

        Atrophic Gastritis Presenting with Pulmonary Embolism

        Atrophic gastritis is an autoimmune gastropathy in which there is destruction of gastric parietal cells. This results in intrinsic factor deficiency and disturbance in vitamin B12 absorption. Its clinical manifestationa are therefore the consequences of B12 deficiency and include anemia and neurological defect. In addition, lack of B12 results in metabolic changes, including disturbances of methionine metabolism and accumulation of homocysteine.

        In recent years, there has been increasing evidence suggesting that hyperhomocysteinemia is a risk factor for thrombo-embolic disease. We describe a 51-year-old man with atrophic gastritis, severe B12 deficiency and hyperhomocystein-emia. The initial clinical manifestation was pulmonary embolism, without either anemia or neurological signs. B12 deficiency should therefore be considered in patients being investigated for hypercoagulability.

        Histoacryl Vs Dermabond Cyano- Acrylate Glue for Closing Small Operative Wounds

        Acrylate glues used in a childrens' day care unit to close small surgical wounds were compared. In 157 children, aged 12 weeks to 3.7 years, either Histoacryl or Dermabond was used (respectively, H: Ethicon Inc., Johnson & Johnson, NJ or D: Dermabond, Braun Surgical Gmbh, Melsungen, Germany). Operations were for inguinal hernia (110 cases), hydrocele (25), undescended testis (16), umbilical hernia (13) and funiculocele (3).

        1 week after surgery the wounds were evaluated in terms of integrity of closure, redness or infection, need for antibiotics, wound granuloma, and parental satisfaction with instructions and actual method of wound caring. 3 months after surgery the wound/scar was reexamined.

        The margins of the wounds were separated partially or completely in 8 of 85 in group H (9.4%) while in the D group, 2 wounds (2.4%) had partially opened (p<0.05). There were no differences between the glues with regard to wound infection or cosmetic results. Parental satisfaction was higher with D (96%) than H (82%) but the difference was not statistically significant.

        It is convenient to use glue to close operative wounds in children after ambulatory surgery. The use of D significantly reduced wound ruptures compared to H. Long-term cosmetic results were similar.

        Advantages of Standardized Protocol for Oral Rehydration in Acute Pediatric Gastroenteritis

        Oral rehydration (OR) for acute gastroenteritis in infants and children has been shown to be as effective as IV therapy, with less discomfort and lower costs. In this retrospective study we compared 2 pediatric wards, in 1 of which only a standardized, simplified, bedside protocol, based on American Academy of Pediatrics guidelines, was used.

        There were no significant clinical characteristics in the 208 patients. In the ward which used the above protocol, OR utilization was significantly more frequent than in the other ward (48% versus 15%), thus saving equipment costs of nearly $1,000/3 months. There were no significant differences in outcome between the wards.

        We conclude that introducing a standardized management protocol may increase OR utilization in hospitalized children with acute diarrhea.

        Coloring the Floor of Schlemm's Canal in Deep Sclerectomy

        Increased intraocular pressure in glaucoma, resistant to maximal tolerated medical therapy, can be relieved by deep sclerectomy. Its advantage over classical trabeculotomy is fewer postoperative complications. This probably results from its being relatively noninvasive, since the anterior chamber is not penetrated.

        Successful deep sclerectomy requires preservation of the trabeculo-descemetic membrane, which forms part of the floor of Schlemm's canal. Aqueous flows from the anterior chamber across the trabeculo-descemetic membrane and into the ocular venous drainage. Despite the importances of its preservation, during deep sclerectomy it is perforated in 10-15% of cases.

        Coloring the endothelium lining the floor of the canal with gentian violet solution improves visualization of the membrane and thus aids in its preservation. Its use makes the procedure easier and should improve results.

        Laparoscopic Cholecystectomy - 1,000 Procedures in a Surgical Department

        The first 1,000 laparoscopic cholecystectomies performed in our department were reviewed. There was no operative mortality; conversion to open cholecystectomy was necessary in 2%. In the last 600 cases the rate of conversion had decreased to 0.5%. There was common bile duct injury in 0.3%, with the injuries identified during primary surgery. This clinical experience is consistent with previous studies, which proved that laparoscopic cholecystectomy is safe and should replace open operation as the procedure of choice.

        Thrombocytopenic Purpura as Sole Manifestation of Brucellosis in a Child

        Thrombocytopenic purpura associated with brucellosis has rarely been described in children. The thrombocytopenic purpura is usually part of the array of manifestations of brucellosis, such as fever, malaise, arthralgia, arthritis, hepatosplenomegaly and lymphadenopathy.

        We describe a 4-year-old girl in whom severe thrombocytopenic purpura was the only manifestation of brucellosis which resolved after appropriate antibiotic therapy. We conclude that brucellosis should be included in the differential diagnosis of thrombocytopenic purpura in areas endemic for brucellosis, and when there is a history of exposure to infected food products.

        Dermatitis from Contact with Agave Americana

        Various plants induce dermatitis in man. There have been only a few published cases of contact dermatitis caused by Agave americana (AA).

        We report intentional exposure to AA in a soldier seeking sick leave, and review our previously reported cases. Treatment with oral antihistamines and topical saline compresses resulted in subsidence of the systemic symptoms within 24h and regression of cutaneous manifestations in 7-10 days.

        Physicians should be alert to the possibility of self-inflicted contact dermatitis induced by exposure to plants, especially to A. americana. Systemic signs may accompany the cutaneous lesions.
