        סרטן הערמונית הוא השאת השכיחה ביותר בגברים מבוגרים. הטיפול בלוקים במחלה מוקדית המוגבלת לבלוטה הוא בניתוח או בהקרנות. בעשור האחרון הוכנסה שיטת הטיפול בקרינה מייננת פנימית – ברכיתרפיה באמצעות גרגירי יוד רדיואקטיביים. אנו מדווחים במאמר זה על 150 חולים ראשונים שטופלו בברכיתרפיה והיו במעקב שנע בין 3-24 חודשים (חציון 15 חודש). בעשרים מהחולים שולבה גם קרינה חיצונית, בהתאם למדדי קבלה מקובלים.

        במעקב שנערך ב-89% מהחולים נצפו ערכי PSA נמוכים מ-0.5 נאנוג'/מ"ל, כעדות להחלמה. רק ב-6 חולים נצפתה עלייה מתונה בערכי PSA – כעדות להישנות המחלה או לאי-החלמה ממנה.

        התחלואה היתה בינונית, והתבטאה בהפרעות בהטלת שתן כתוצה מחסימה ובגירוי (irritability), שנמשכו כ-3 חודשים בממוצע. לאחר תקופה זו, ערכי ה-IPSS – ניקוד לתסמינים של מוצא השלפוחית – חזרו לערכים טרם הטיפול.

        שליש מהחולים התלוננו על אין-אונות, שהופיעה רק לאחר הטיפול. הערכים נמדדו באמצעות שאלון למילוי עצמי (IIEF). החולים לא לקו בסיבוכים קשים, ולכן לא היו זקוקים לניתוח, אישפוז או צנתור ממושכים, ולא הובחנו נזקי קרינה באזור החלחולת.

        לסיכום, במאמר זה מדווח לראשונה בישראל על שיטת הטיפול בברכיתרפיה באמצעות יוד-125 בלוקים בסרטן הערמונית. תוצאותיה לטווח קצר ולטווח בינוני מצויות במתאם עם הדיווחים בספרות, ופגיעתה באוחר-החיים של החולה היא מיקטית (minimal), דהיינו הפגיעה בהרגלי ההשתנה והזיקפה של החולים היא נמוכה (רק בשליש מהחולים) ובהשוואה להשפעות-הלוואי והסיבוכים המדווחים בטיפולים חלופיים, כגון ניתוח רדיקלי או קרינה חיצונית.

        Computed Tomographic Angiography of the Peripheral Vasculature - Role & Applications in Vascular Surgery

        Computed tomographic angiography (CTA) is a relatively new diagnostic modality in the field of vascular surgery. Despite being new it has already been introduced into a wide range of diagnostic applications in this field and in many cases it can precede or replace the conventional intra-arterial angiography. During a 12 month period between 1.8.98 and 1.8.99 sixty five peripheral arterial imaging scans were performed using a CT Twin - 2 helical scanner (Spiral Twin Flash., Elscint, Israel) with a 100% technical success rate and no complications at all. Twelve patients (18.5%) were operated upon and 20 (30.7%) underwent endovascular procedures with full intra-procedural agreement with the pre-operative or pre-procedure CTA findings. Despite possible pitfalls and a few disadvantages the technique carries major benefits and significant advantages to both the patient and the clinician. Therefore we recommend considering CTA as a first line diagnostic modality whenever peripheral vasculature has to be demonstrated whether electively or urgently and to spare the conventional angiography for selected cases only.

        שיעור האלימות בחברה המערבית בכלל ובישראל בפרט נמצא בעלייה מתמדת, והפך להיות גורם משמעותי לתחלואה ותמותה. האלימות מהווה בעיה חברתית, חוקית וסוגיה בבריאות הציבור. בחודשים האחרונים, מדווח בעיתונות הכתובה והאלקטרונית על מעשי אלימות ורצח של חולי נפש. בחלקם היו החולים בקשר כלשהו עם שירותי בריאות הנפש בעברם. בנוסף, בחלק מהמצבים בהם ננקט פשע אלים, טוען הנאשם לאי-שפיות כהגנה משפטית. בסקירה זו, נדון בשאלה האם חולי נפש בכלל וחולי סכיזופרניה בפרט אלימים יותר משאר האוכלוסייה, והאם הפשע שמבצע חולה הנפש שונה באיפיוניו מפשעים המבוצעים על ידי עבריינים שאינם חולים.

        Breaking Bad News - A Structured Course for Primary Care Physicians

        Physicians are frequently required to break bad news to their patients. Previous research has shown that inconvenience, incompetence, and difficulty in dealing with patients' feelings are the main complaints expressed by physicians after such an encounter. Current educational programs dealing with breaking bad news are usually short, given in lecture format, and are inadequate in addressing essential issues such as knowledge, personal beliefs and attitudes, and previous personal experiences of physicians in such situations.

        In the past 8 years our Dept. of Family Medicine has implemented a course in breaking bad news that addresses these issues. A senior family practitioner and a medical social worker conduct 14 sessions of discussions and role-playing for small groups of residents and primary care physicians. The program is based on: theory dealing with methods of managing stress and crisis intervention, clarifying personal attitudes, discussions of previous personal encounters of the participants, various modalities of communication, methods of addressing patients' feelings and emotions, and coping with the emotions of the one breaking the bad news.

        On a 1-5 Likert scale questionnaire the course received an overall score of 4.47 (SD 0.51). Participants noted that they gained relevant communication skills for future patient encounters. A reliable examination of practitioners' competence in breaking bad news is mandatory in order to assess the efficiency of such courses.

        Testicular Cancer: Self-Awareness and Testicular Self-Examination in Soldiers and Military Physicians

        Testicular cancer is the most common malignancy in young men. To evaluate knowledge and awareness of that cancer, and of the practice of testicular self-examination (TSE), we developed a questionnaire which was distributed to 717 male soldiers and 200 of their military physicians.

        21% of the soldiers had received some explanation of the importance of TSE, but only 16% were actually instructed how to perform TSE, and only 2% practiced it regularly. 24% had never examined their testicles before, 185 only rarely, and 6% often. With increased age, TSE frequency increased, but previous education, type of military unit, and ethnic origin had no affect. 99% of military physicians had been taught how to examine breasts, but only 70% had been taught routine testicular examination. 22% performed it, but 27% never did. 84% had never taught their soldiers the importance of TSE, although 51% taught female soldiers breast self-examination.

        There was a significant lack of awareness of the importance of regular practice of TSE among both soldiers and their army physicians.

        Elective Repair of Infra-Renal Aortic Aneurysm

        Age over 80 years is generally considered an independent risk factor in elective surgery for abdominal aortic aneurysm (AAA). As the general population increases in age, more elderly are likely to be candidates for such surgery.

        We studied prospectively 100 consecutive patients undergoing elective AAA surgery between 1992-1995. All were operated on by the same team of anesthetists and surgeons and all were transferred to the general ICU for at least the first 24 hours. 16 were above the age of 80 (Group I) and 84 below (Group II).

        We recorded preoperative factors (demographics, medical history, risk factor indices, EKG findings, as well as left ventricular ejection fraction (LVEF) and stress imaging when indicated); intraoperative factors (duration of surgery, size of aneurysm, complications and units of blood transfused); postoperative factors (length of ICU stay, duration of ventilation, APACHE II [Acute Physiological and Chronic Health Evaluation] and TISS [Therapeutic Intervention Scoring System] scores; complications in the ICU, need for readmission to the ICU, and mortality).

        In Group I LVEF was greater (p=0.03) and aneurysm size significantly larger (p=0.036), but there were no other significant differences between the 2 groups with regard to pre- and intraoperative data. Group I patients were not ventilated as long (p=0.038), but there were no significant differences in outcome factors. Mortality for the whole group was 5% and was not significantly different in the 2 groups (1/16 in Group I and 4/84 in Group II).

        We conclude that there is no excess morbidity or mortality in octogenarians undergoing AAA surgery. However risk of the aneurysms rupturing is significantly greater since they are larger. We suggest that age not be considered the sole criterion for aneurysm repair, or at least not in selected patients with normal LVEF.

        Severe Transient Neutropenia due to Parvovirus B19

        A 10-year-old girl was admitted with a 3-day history of fever, cough, abdominal pain and vomiting. Severe neutropenia (total neutrophil count 186/mm³), a mild increase in ALT and AST, and a positive titer of IgM antibodies against parvovirus B19 were found. The neutropenia resolved and liver enzymes became normal as she recovered. We conclude that parvovirus B19 infection should be considered in the evaluation of an acute illness accompanied by severe neutropenia.

        The New Family Physician

        The content of all consultations of a family physician during the first 2 months in a new practice and again in the same 2 months, 2 years later were analyzed. The workload did not change between the 2 periods, but in the first period there were relatively more men over the age of 60 years, and a more marked predominance of women in the 45-59 year age-group.

        There were more follow-ups of pre-existing conditions during the first period, but in both periods the main reason for visits was an acute condition; a quarter of visits required only simple counseling without further investigation or prescribing of medication. During the first period 9% of visits were for the explicit purpose of getting to know the new physician, usually by women.

        Thus the initial period in a new practice is not excessively burdensome, but involves seeing a number of patients who come either simply to get to know the new doctor, or to present non-chronic problems without expectation of further investigation.

        Multisystem Disease Caused by BCG Imitating Miliary Tuberculosis

        As the prevalence of tuberculosis is on the rise in western countries, we present a 79-year-old man who developed a pulmonary tuberculosis-like syndrome following immunotherapy with BCG for carcinoma of the urinary bladder. The symptoms subsided following 3-drug antitubercular treatment, and the addition of steroids following negative cultures for Mycobacterium tuberculosis. The course of this disease, named BCG-osis, is much more favorable than miliary tuberculosis, even with milder treatment. It is important to keep in mind this phenomenon now that there is increasing treatment of cancers with BCG.

        Influenza Surveillance through Sentinel Reporting Clinics

        In a joint effort of the Israel Center for Disease Control, the National Center for Influenza in the Central Virology Laboratory, together with a group of collaborating pediatricians and family physicians, a network for influenza surveillance was established in the winter of 1996-97. Nose and throat swabs were obtained from 571 patients with flu-like illness. 133 (23%) were positive for influenza virus. Both influenza A(H3N2) and B were isolated, predominantly influenza B during the beginning of the season. Both circulating strains were antigenically similar to those included in the vaccine for 1996-1997. Patients from whom influenza virus was isolated were significantly more likely to suffer from cough and myalgia in comparison with patients whose cultures were negative (p=0.02 and 0.003. respectively). Results of the first year of surveillance indicate that sentinel reporting clinics are useful for timely detection and identification of the viral strains circulating in the community, thus allowing prompt intervention in preventing the spread of influenza. Conclusions from the first year of the study were drawn and applied in the winter of 1997-1998.

        Atypical Infantile Hypertrophic Pyloric Stenosis

        Infantile hypertrophic pyloric stenosis (IHPS) is the most common reason for nonbilious vomiting in infants. Its cause is unknown. Hypertrophy of pyloric muscle can progress after birth and reach complete gastric outlet obstruction. Usually symptoms start after the age of 3 weeks. In the past diagnosis was based on history of projectile, nonbilious vomiting and palpation of a pyloric mass. Greater awareness of IHPS and increased use of imaging modalities, mainly abdominal ultrasonography, have resulted in a change in the clinical condition at presentation. The length of illness before admission has decreased and weight loss, dehydration and metabolic abnormalities have become less common. We describe an atypical clinical manifestation of IHPS: bilious vomiting. This atypical presentation may be due to earlier diagnosis.

        Meningitis Due to Streptococcus Bovis Type 2

        Meningitis due to Streptococcus bovis is rare. Only 14 cases having been reported in the English literature. All patients (including the patient described) had an underlying disease or were treated by pharmacological agents that predisposed the patient to the infection. Most were treated by monotherapy with penicillin G (or amoxicillin) and recovered.

        We describe a 74-year-old woman who had splenectomy as treatment for hairy cell leukemia 6 months before hospitalization for meningitis and sepsis by S. bovis type 2. She was successfully treated with intravenous amoxicillin. There was neither evidence of endocarditis nor carcinoma of the colon. Although the association between S. bovis meningitis and endocarditis or carcinoma of the gastrointestinal tract is not well established, we recommend a full work-up for GI malignancy and endocarditis in every patient with S. bovis meningitis.

        Second Degree A-V Block in Graves© Disease

        Cardiac tachyarrhythmias are common in thyrotoxicosis. We report an uncommon case of a 53-year-old woman with Grave's disease who developed second degree A-V block while euthyroid on propylthiouracil. The most likely mechanism is an autoimmune response causing infiltration of the cardiac conduction pathways.

        Abnormal Liver Function Tests in the Primary Care Setting

        Results of laboratory tests ordered during a primary care encounter may reveal findings of abnormal liver function tests, including elevated liver enzymes, hyperbilirubinemia, hypoalbuminemia or abnormal coagulation tests. The object of this study was to describe the spectrum of these liver function test (LFT) abnormalities in primary care.

         

        Results of all laboratory tests ordered during 10 months in an urban primary care clinic were retrospectively reviewed and the medical charts of patients with abnormal LFTs were studied. In 217/1088 (20%) of the tests at least 1 LFT abnormality was found in 156 patients. New diagnoses were made in 104 patients. The main diagnostic groups were: non-alcoholic fatty liver changes, Gilbert's disease, acute infectious hepatitis, alcoholic liver disease and cirrhosis and hepatotoxic drug injury. In 60 patients the physician classified the abnormality as negligible and not associated with significant disease. However, an abnormal test that had been ordered for evaluation of a specific complaint, was indeed likely to represent significant disease (X²=29.5, p<0.001). We conclude that finding abnormalities in liver function tests is common in the primary care clinic but does not often indicate significant liver disease.

        Fatal Asthma in Children: Preventable?

        Sudden death from asthma is rare but occurs in the young age group. We recently faced this rare situation, when 3 asthmatic children were dead on arrival at the local emergency room. All 3 had been treated with beta-2 agonist inhalation on a regular basis, without anti-inflammatory treatment. 2 of the children died while inhaling the beta-2 agonist. It is important that there be clear guidelines and full education about the management of asthma, during and between exacerbations, to prevent such deaths.
