        ההודעה של ה-FDA על מתן אישור לתרופה קטיגריס (xigris) של חברת לילי לטיפול באלח דם קשה, היא מהחדשות המרשימות ביותר הנוגעות לטיפול בחולים במצב קשה. הודעה זו באה לאחר פירסום תוצאות מחקר רב-מרכזי, כפול סמיות שלב III, שנערך מטעם חברת לילי ופורסם ב-New England Journal of Medicine בחודש מרס 2001. במחקר זה נמצא, כי התרופה קטיגריס מפחיתה את שיעור התמותה מאלח דם ב- 6% - מ-30% ל-24%.

        אלח-דם (sepsis) הוא התגובה המערכתית לזיהום על ידי גורמים שונים. הלם אילחי הוא ביטוי קשה יותר של אותה תיסמונת. לעתים קרובות אירוע של אלח-דם או הלם אילחי יופיעו בחולים עם מצבים רפואיים קשים, כמו לאחר פגיעה קשה או מחלות קודמות, ושכיח מאוד שאלח-דם יתהווה לכדי הסתמנות קריטית המחייבת ניטור וטיפול תוקפניים. הביטוי המערכתי לזיהום יכול להופיע בעקבות זיהום חיידקי, פטרתי, נגיפי או טפילי, אך לרוב מדובר באלח-דם על רקע זיהום בחיידקים, בבתי-חולים עולה השכיחות של אלח-דם על רקע חיידקים עמידים לאנטיביוטיקה מסוגים שונים נוכח הטיפול הנפוץ באנטיביוטיקה רחבת-טווח בחולים מאושפזים. עובדה זו מעלה גם היא את החשיבות של טיפול המכוון כנגד התהליך האילחי עצמו בקרב חולים שלוקים באלח-דם מאטיולוגיות שונות.

        מלריה ממשיכה להיות סכנה משמעותית בקרב הנוסעים למדינות אנדמיות. ההקפדה על נטילת הטיפול המונע וההגנה מפני עקיצות יתושים. הן הדרכים היעילות ביותר למניעת המחלה.

        Prenatal Down's Syndrome Screening at 10-14 Weeks Gestation using the Combined Nuchal Translucency and Maternal Serum Biochemistry: Preliminary Results

        We report our preliminary experience of prenatal screening for Down's syndrome (DS) using nuchal translucency (NT) measurement combined with the serum biochemistry analysis of Free b-human chorionic gonadotropin (FbhCG) and pregnancy associated plasma protein A (PAPP-A) all measurement at 10-14 weeks of gestation.

        Of the 358 parturient women which enrolled in the study, 9 cases were not included because of fetal anomalies or miscarriages. Thus the study group included 349 singleton pregnancies in which complete prenatal and infant follow-up was available.

        Forty-four pregnant women were found to be screen positive (12.6%) and in 13 cases (27%) of them fetal chromosomal aneuploidies were diagnosed. Looking into the markers profile we found that the NT was a sensitive marker which was abnormally increased in all the fetal aneuploidies. Serum FbhCG was found to be a promising marker as well, being significantly elevated (2.26[0.86 multiple of the medians, MoM) in DS cases, and decreased (<0.5 MoM) in two cases of Edward's syndrome.

        On the contrary, PAPP-A was found less sensitive, and its mean MoM values were not significantly different between DS versus euploid fetuses.

        Our preliminary results support the promising success of DS screening using NT and FbhCG.

        Gastrointestinal Hemorrhage of Obscure Origin

        Gastrointestinal bleeding of obscure origin consists of recurrent bouts of acute or chronic bleeding for which no definite source is discovered in routine endoscopic and barium contrast studies of the upper and lower gastrointestinal tracts. Usually its cause is angiodysplasia of the intestine, but many cases are due to tumors, mostly of the small bowel, which may be malignant.

        In patients under the age of 50, the proportion with malignancy is relatively high (up to 14%) as compared to older patients. We describe a 45-year-old woman who suffered from gastrointestinal bleeding for 3 years. The cause of bleeding was not found despite extensive work-up.

        In her last admission for acute gastrointestinal hemorrhage she was given a total of 30 units of blood. A tumor of the small intestine found by angiography was excised and found to be a stromal tumor of uncertain malignant potential. 1 year after operation she is asymptomatic without bleeding and her hemoglobin is stable without treatment.

        Prolonged Neuromuscular Damage following Cortico-Steroids and Muscle-Relaxants

        Many patients mechanically ventilated for acute respiratory failure, are treated with medication that includes a combination of cortico-steroids and non-depolarizing neuromuscular-blocking agents (NNBa). A third of them can be expected to develop delayed neuromuscular damage, which may be severe and prolonged.

        We describe a 50-year-old man who suffered from acute myeloid leukemia and was ventilated due to pneumonia. He was treated with pancuronium and cortico-steroids, and during recovery suffered quadriparesis that lasted several months.

        Typically this damage is purely motor and is accompanied by absent tendon-reflexes, sometimes with elevated creatin-kinase. Muscle biopsy usually shows deletion and degeneration of thick myosin filaments. The phenomenon is related to the duration of NNBa treatment, and probably results from an adverse synergistic effect on muscle tissue of the cortico-steroids and cortico-steroid-like NNBa given the immobilized patient.

        Awareness of this adverse effect of steroids and pancuronium, the use of passive mobilization, shortening the use of NNBa and early rehabilitation would minimize disability due to this phenomenon.

        Carbon Dioxide Gas Embolism during Laparascopic Cholecystectomy

        A case of carbon dioxide gas embolism during laparascopic cholecystetomy is presented. Prompt diagnosis and immediate treatment resulted in positive outcome. Laparascopic cholecystectomy is nowadays one of the most common operations for cholecystectomy and certainly the most frequent endoscopic surgical procedure. It is usually safe and effective. However, a number of serious complications have been reported during the procedure.

        Gas embolism is one of the complications that may occur during the initial gas insufflation or during dissection of the gall bladder. Symptoms are mainly related to the speed and the amount of gas that reaches the venous system. Early recognition and prompt treatment are required to prevent severe morbidity or even fatal outcome.

        Transesophageal echocardiography has detected many unsuspected cases of gas embolism. However, capnograph monitoring of end-tidal CO2, routinely used in everyday anesthesia, is highly reliable in alerting to the possibility of gas embolism and also in confirming its occurrence.

        In cases of suspected gas embolism close collaboration between anesthetist and surgeon is required.

        Massive Pulmonary Embolism

        Pulmonary embolism is a common event in hospitalized patients. In some cases it presents with hemodynamic collapse, indicating massive obliteration of the pulmonary vasculature and has a very grim prognosis; 2/3 of such patients die within 2 hours of onset of symptoms. We describe our experience in 13 patients with massive pulmonary embolism. An aggressive diagnostic and therapeutic approach, utilizing sophisticated imaging techniques, thrombolytic therapy and surgery, led to the survival of 8 of the patients. Our experience supports an aggressive approach in these seriously ill patients.

        Kimura's Disease and Angio- Lymphoid Hyperplasia

        Kimura's disease is a rare angiolymphoid proliferative disorder of soft tissue characterized by subcutaneous swelling and a predilection for the head and neck. There are usually enlarged regional lymph nodes, eosinophilia and elevated sedimentation rate and IgE levels. A 26-year-old women with subcutaneous masses in the submandibular area is reported. The diagnosis of Kimura's disease versus angiolymphoid hyperplasia is discussed.

        Closed Intramedullary Nailing of Forearm Fractures in Young Patients

        Forearm bone fractures are commonly the result of falling on the outstretched hand or of direct injury. The preferred treatment is debated. The possible modalities are: application of a cast, often necessitating repeated manipulations; open reposition and fixation by plates and screws; or closed manipulation and closed intramedullary nailing. We present our favorable results in treating these fractures in young patients by closed intramedullary nailing, and compare them with the unfavorable results of this method in mature adults.

        Hemato-Oncology Patients in Acute Respiratory Failure in the ICU

        Hemato-oncology patients needing mechanical ventilation for acute respiratory failure (ARF) have an extremely poor prognosis, with a mortality of more than 90%. Over an 18 month-period 17 such patients were admitted to our ICU. Diagnoses included leukemia (11 cases), lymphoma (1), and status post bone marrow transplantation for leukemia, lymphoma or breast cancer (5). Of 8 whose ARF was associated with septic complications due to neutropenia following chemotherapy, 6 survived. Of 9 who developed ARF due to toxic damage to vital organs following high-dose chemotherapy, 2 survived. Those who develop ARF during chemotherapy are expected to have an increase in granulocyte count within days, and have a surprisingly good prognosis. They should be admitted to the ICU and treated aggressively. Those who develop sepsis due their primary disease and whose general condition contraindicates chemotherapy, have an extremely grave prognosis and admission to the ICU may not be warranted.

        Immediate Orthopedic Surgery in Multiple Trauma

        The increase in mortality and morbidity from multiple trauma due to road accidents, industrial trauma and combat injuries obligates treatment based on emergency systems and trauma centers. Follow-up of the frequency of different types of injury calls attention to increasing involvement of the orthopedic surgeon in primary treatment. This situation calls for appropriate deployment of immediate surgical treatment which will rapidly enable mobility. We present several methods for immediate orthopedic treatment of multiple-trauma patients.

        Water Intoxication following Desmopressin Overdose

        Water intoxication is a serious condition which may be caused by desmopressin overdose, with reversible or irreversible neurological complications. In the past, desmopressin was used in endocrinological centers for the treatment of anti-diuretic hormone deficiency (central diabetes insipidus). Indications for hormone treatment have since widened, especially as an effective solution for nocturnal enuresis. It is now often prescribed in community clinics, and its use has been encouraged by extensive promotion. We describe a 15-year-old boy with primary nocturnal enuresis who started treatment with desmopressin 1 year prior to admission. He was allowed to use the drug without supervision, and drank excessively. The result was water intoxication which required admission for intensive care because of loss of consciousness and convulsions for 36 hours.
